I, ________________________________________________________, (hereinafter referred to as the "Client" or the "Subject") hereby give my consent to DIADA Limited Liability Company (hereinafter referred to as the "Operator") for the processing of my personal data as listed in Clause 2 hereof under the following conditions:

1. The Subject hereby consents to the processing of his/her personal data by the Operator, which means the performance of the following actions: collection, systematization, accumulation, storage, clarification (updating, modification), use, distribution (including transfer), disclosure to third parties, depersonalization, blocking, destruction of personal data, with the above mentioned methods of data processing described in Federal Law No. 152-FZ dated July 27, 2006 "On Personal Data", for the following purposes:
· Provide services to Customers;
· Conclude and further fulfill contracts with Customers;
· Transfer Personal Data to third parties, if necessary for the fulfillment of the agreements concluded between the Operator and the Subject, provided that such third parties maintain confidentiality and security of Personal Data;
· Provide the Subject with advertising information by phone or e-mail, information on goods and services sold by the Operator and/or its partners, as well as for the Operator or its partners to make a decision on the applicability of loyalty programs, discounts and other preferential conditions to the Subject, and inform the Subject on such decisions made;
· Receive information from the Operator by phone or e-mail, including that to submit inquiries at https://www.litz.pro.

2. List of personal data submitted for the Operator for processing:

• surname, first name, patronymic;
• contact details (e-mail address, phone numbers);
• anonymized visitor data (including cookies) using Internet Statistics Services (Yandex.Metrica, Google Analytics, etc.).

3. Clause 4 of Article 14 of Federal Law No. 152-FZ dated July 27, 2006 "On Personal Data" states that the Personal Data Subject upon his/her written request shall have the right to stay informed of the processing of his/her personal data.

4.This Letter of Consent shall come into force once submitted by the user of the website https://www.litz.pro and shall be terminated either upon written application (withdrawal) of the user of the website https://www.litz.pro under Clause 2 of Article 9 of Federal Law No. 152-FZ dated July 27, 2006 or upon achievement of the personal data processing purposes.

5. The Operator shall have the right to perform the following actions with the above personal data:

• collection;
• systematization;
• accumulation;
• storage;
• clarification (updating, modification);
• use;
• distribution/transfer;
• blocking;
• destruction;
• depersonalization;
• any other actions under the applicable laws of the Russian Federation.

These actions may be performed with or without the use of automation tools.

6. This Letter of Consent for Personal Data Processing may be withdrawn by submitting an application (withdrawal) through a notary public or by registered mail with notification of delivery, or by delivering the application against receipt. If the Personal Data Subject withdraws his/her consent to the processing of personal data, the Operator shall be entitled to continue the processing of personal data without the Personal Data Subject’ consent upon existence of the grounds specified in Clauses 2–11 of Part 1 of Article 6, Part 2 of Article 10 and Part 2 of Article 11 of Federal Law No. 152-FZ dated July 27, 2006 "On Personal Data".

7. If the Subject revokes his/her personal data, the Operator shall be entitled to continue storing personal data in order to provide information in response to requests from state bodies, officials, courts of general jurisdiction, arbitration courts, law enforcement bodies and other entities conducting internal audits on the legitimacy of personal data processing by the Operator.

I acknowledge that I have read and understood Federal Law No. 152-FZ dated July 27, 2006 "On Personal Data", with the Operator's Personal Data Processing and Security Policy (available at https://www.litz.pro), my rights and obligations related to personal data protection, including the potential consequences of my refusal to give consent to the processing of personal data.
By submitting a request for a callback at https://www.litz.pro (by ticking the box "I consent to the processing of my personal data") the Subject gives his/her consent to the processing and distribution of personal data.

I acknowledge that by giving such consent I am acting of my own free will and in my own interests.
